Chase Memorial Nursing Home & Rehab to host community event celebrating quality service achievements

NEW BERLIN – The Chase Memorial Nursing Home and Rehab, a member of Good Shepherd Communities, is pleased to announce a Town Hall Celebration to honor the facility's outstanding accomplishment of achieving 4-stars for Quality Measures from the Centers for Medicare and Medicaid Services (CMS).

The event will take place on Tuesday, November 12 at 2:30 p.m. at Chase Memorial Nursing Home and Rehab in New Berlin.

"On behalf of the Board of Directors of Good Shepherd Communities, we would like to congratulate Patti Hitchcock and her staff on achieving a 4-star rating at Chase Memorial. This is a major accomplishment to be proud of and shows the dedication Chase Memorial staff has to meet the needs of its residents,” said Rick Allabaugh, Chair of the Good Shepherd Communities Board of Directors.

“I am proud of this accomplishment for Chase Memorial Nursing Home and its staff," added Jason Santiago, President and CEO of Good Shepherd Communities.

"For over six decades, we have provided excellent care to our residents. Chase stands out as a small nursing home in the tight knit community of New Berlin. Our staff have known these residents long before they arrived at Chase. I have seen firsthand how caring and compassionate our staff are to residents and families. Patti Hitchcock, Executive Director, and the Chase Staff have done a tremendous job earning this CMS 4-star designation,” he said.

Hitchcock commented, "I am extremely proud to see Chase Memorial recognized with a 4-star quality rating, a reflection of our team's dedication to delivering safe, compassionate, and effective care every day. While our facility rating stands at 3 stars, we view this as an opportunity for continued growth and improvement. We remain committed to enhancing our environment and services to meet the highest standards for our residents and their families."

She said the 4-star rating reflects Chase's commitment to clinical excellence and superior resident care. Ratings are based on data submitted by the facility through regular reporting cycles, including clinical outcomes, staffing levels, and health inspection results. Facilities are evaluated on both quality measures and stmctural performance, with ratings updated quarterly.

The rating specifically evaluates outcomes related to how well a facility supports residents' physical and clinical well-being, key areas such as reducing hospital readmissions, preventing falls, and improving overall health management. Chase Memorial has implemented targeted initiatives to improve care coordination, reduce hospital readmissions, and strengthen staff training, all of which contributed to the facility's improved quality score.

The Department of Health calculates ratings using standardized CMS methodology, drawing from inspection reports, resident assessments, and staffing data. These ratings are intended to provide transparency and help families make informed decisions. The DOH also reviews and validates submitted data to ensure accuracy and consistency across facilities statewide.

In addition to the 4-star rating, Chase Memorial recently advanced to an overall 3-star rating, marking a significant milestone for staff, residents, and the entire Good Shepherd Communities network.
